Drug Testing

TLC Policy

• Fee for services are assessed at intake and are to be paid entering the therapy service unless payment arrangements have been made with office manager. All services rendered muse be paid before receiving completion certificate.

• All Child Welfare consumers who have Medicaid who have Title 19 or Healthy Adult Expansion Program will have their services paid through Medicaid. Consumers will be responsible for any co-pay per service.

• Drug Court consumers will be responsible for their co-pay per service

• You must be SOBER and DRUG FREE at all appointments

.

• Your confidentiality is priority at TLC. A release of information is required to be signed for ANY outside communication by TLC.

• TLC does not participate in Oklahoma’s health information exchange program.

• TLC abides by federal laws HIPAA and 42 CFR part 2

TLC Policy cont.

• Violence and threats will be prohibited.

• The only absences will be by person basis.
  

• All missed sessions unless made up are reported to referral source.
  

• Drug Test: Consumers are assigned a color at intake if you are referred by DHS Child Welfare and /or TANF. Self referrals for testing are arranged at intake
  

• Consumers are expected to dress appropriately. No drug, alcohol, or vuglar graphics allowed.

• Children can never be left unattended and may not attend your session , unless it is a family session.
  

• Cell phones are to be turned off before entering the TLC office.
  

• All Drug Court consumers are expected to attend all court dates and complete all phases.
  

• TLC has grievance forms available if you wish to file a grievance. These forms are located in the front office and in your handbook given to at intake.
